If smoking is 18+, then why aren't firearms and nudity also 18+?

This is now marked 18+. I assume it was not before.

From conversations on other 18+ puzzles, I can tell you that it is the moderators who put on the rating, not the artists. If you see a puzzle not marked that you think should be, contact them.

Nudity is marked 18+.
