I had no idea what the hell was going on in this pic until I saw the title. The man is fairly visible even if the stethoscope isn't recognizable as one, but the woman is a mess.

Most of these crosswords are designed by Russians and you know a Russian lady won't go out (even to the doctor) unless she's dressed like a princess! (joking) Yeah, I thought the black thing was a snake until I read the title...

The woman was easy, but since I was convinced the black was a beard I had to finish the whole thing to realize what was going on.
