Because of the name of the character, most likely...this is 'Big Gay Al' from the South Park series.
Makes no sense, but Keith might be on to the reason... I certainly didn't expect it to get an 18+ rating!

Actually, now that I think about it... it may be because he is holding a cigarette; they put anything involving tobacco as 18+ as far as I can tell.
